The Citizen’s “Personal Electronic Health Record” myHealthKard mobile application serves as a comprehensive repository of a citizen’s EHR that includes their medical history, present and past conditions, immunization records, treatments, and current episode of care.
Personal Electronic Health Record (EHR) mobile application records a citizen’s health information, including allergies, illnesses, vaccination and immunization records, medical encounter history, adverse event reactions, and diagnostic services. It serves as a comprehensive repository for all of their health-related data.
Personal Electronic Health Record mobile application uses a QR (Quick Response) Code to identify a patient based on the “Scanova” or the EMV QR CODE mandated by the BSP Circular Letter no. 2019-080 Philippines Payment Management Inc. (PPMI) as the National QR Code.
The QR code includes information on the vaccines and the dates of the jabs given to the patient.

To comply with Data Privacy Laws, patient consent is required before any data sharing can occur. Our mobile app includes this requirement to ensure patient privacy and confidentiality.
Citizens have the option to click on the “ALERT” button in the myHealthKard app to instantly send their Personal Information and report any Adverse Events to the Vaccination Control Center.

The services provided by FindMe, such as TeleConsult, Admission, and Ambulance, are available online and allow patients to easily book appointments and make payments using the KonekPAY mobile app for a faster and more convenient experience.
myHealthKard allows patients to book medical appointments with Doctors, Hospitals, Clinics, Treatment Centers, Diagnostic Centers and Ambulance Services.
When booking an appointment, patients can view the exact available schedule of Doctors and the available facilities of hospitals are displayed for easy scheduling of their appointments.
Through myHealthKard, patients can easily connect to KonekPAY Mobile for fast and secure online payment transactions.
KonekPAY Mobile is a web application that offers contactless payment options, allowing payers to receive payment bills via SMS notification. Within KonekPAY Mobile, payers can view billing information and choose their desired payment option, including GCash and PayMaya.
Through myHealthKard, FindMe offers patients the option of both face-to-face and teleconsultation with healthcare providers. Teleconsultation allows Patients to connect with Consulting Physicians virtually, providing a safe and convenient option during times of pandemic or when in-person consultations are not feasible.

These Healthcare Providers are grouped by Regions, Province, and City/Municipality. myHealthKard app can be downloaded for FREE from both Google Play and Apple Store.
Aside from the Passport, myHealthKard also utilizes QR code (Quick Response Code) technology to store Citizens’ Information, which can be used to validate and authenticate ownership of the myHealthKard app.
The QR code also has an “error correction” built-in to ensure that the records are authentic.
